Coloplast Titan vs AMS 700 Penile Implants for Erectile Dysfunction: Which Is the Best For You?

are you thinking about a penile implant for your erectile dysfunction well I'm Dr Rena Malik urologist and pelvic surgeon and today we're going to compare two of the most popular penile implants on the market before we delve into the difference between the coloplast and the Boston Scientific penile implants let's quickly review erectile dysfunction erectile dysfunction refers to the consistent inability to achieve or maintain an erection that's sufficient for sexual intercourse meaning that you can penetrate vaginally and it will sustain firmness until the end of the ACT if you ejaculate before you're ready that's not erectile dysfunction that is premature ejaculation now Ed can have various causes like vascular disease and that's most often due to underlying health conditions like high blood pressure or diabetes or neurologic conditions or even prior surgeries that can affect the nerves to the penis also hormonal problems like low testosterone certain medications and even sometimes psychological factors so it's important to see your doctor and make sure you've identified if any of these things are reversible I've also made a full video on erectile dysfunction and all wrong that can be corrected and nothing's working penile implants can be a viable option these are devices that are surgically implanted that create a natural looking and functional erection these are not meant to increase penile length or girth the key word here is function if you are looking at your penis and you stretch it maximally that's the length you're gonna get when you get a penile implant it's not going to get any longer it's also not going to get any girthier than your Anatomy a lot now let's get down to it and compare the two most popular implants the coloplast Titan and the AMS 700 both are made up of a pair of cylinders those are inserted into to the penis along with a small pump that's placed in the scrotum when they're activated the pump allows you to achieve an erection on demand and a release valve then helps you return to a flaccid State when you're done so no one can really tell that you have an implant the AMS 700 implant is made up of silicone whereas the coloplast Titan is made up of bioflex they're both made of materials that are inert so the body forms kind of a protective barrier around them preventing the body from having a reaction so that you can have them and use them and the body doesn't eject them from your body the AMS implant comes in three variations the lgx the CX and the cxr and this is essentially describing the variation in the layers so the AMS is made up of three layers it has an inner and outer layer with silicone and the middle layer is Dacron Lycra this is like a weaved Fabric and it can be in One Direction which is what you'll find in the CX and the cxr meaning that it only expands in girth or it can be into directions like the lgx which allows for length and girth expansion also the other beauty of the design of the AMS is that three layer design makes it feel the most natural when it's deflated so it's completely comfortable and it's very hard to tell that it's there the CX has controlled expansion in girth meaning that it can get really really firm the cxr is specific where people have very narrow penises or they might have more scarring or other abnormalities the lgx as I mentioned earlier allows for length and girth expansion now this sounds great but it only comes in certain sizes and is usually better for people who have shorter phalluses whereas the CX allows for a more firm girthy appearance now the coloplast Titan as I mentioned is made up of Pop bioflex which is polyurethane now this is a strong material that allows it to expand in girth meaning that it can get wider when it's activated it also comes in a narrower cylinder as well as the Titan and the narrow cylinder against people who have narrow penises or have a lot of fibrosis or Scar Tissue now let's talk about the variations in these two implants first let's start off with coding the coloplast Titan is coated with what's called a hydrophilic coating this means that it can absorb fluids specifically fluid-based antibiotics and the important part of these implants is that they can get infected so we want to do everything we can to prevent infection so the coloplast hydrophilic coating allows you to dip the coloplast implant into whatever antibiotics you want and that will take it up and allow it to kind of coat the implant and the beauty of that is your surgeon can pick whatever antibiotics they want the AMS 700 series implants are coated with what's called inhibit Zone and that is a specific antibiotic coating which is Rifampin and Minocycline and so when this is implanted these antibiotics release over days into your tissues to avoid infection now both of these have a allowed implants to be very very safe in fact infection rate is lower than about five percent and despite the differences when you're using appropriate antibiotics like Rifampin and Gentamicin on the coloplast Titan versus the AMS there's been no difference in infection rate between the two implants now let's talk about the release pump so the coloplast release pump is unique because it's got a softer bulb which makes it really easy to pump up it also has a One Touch button that you need to deflate the implant so usually you'll touch the button squeeze the cylinders and the fluid will then go back into the reservoir to allow deflation now the AMS pump has what's called a momentary squeeze pump compared to the coloplast pump this one may be a little bit more difficult to pump up but really really easy to deflate also when you press the button to deflate it vibrates which allows you to then really be sure that you're pressing the right thing both devices have things that prevent Auto inflation meaning that the fluid can't go from the Reservoir into the penile implant when you don't want it to now rigidity so let's talk about the implant in the inflated state in studies they found that the Titan implant has slightly increased axial rigidity and this is because it's made of bioflex which is a very strong polymer so it showed specifically with longer phallus increased rigidity or in patients who have mild Peyronie's disease also showed increased rigidity compared to the AMS implant now Peyronie's disease is a curvature of the penis due to plaque if you want to learn more about that I've made videos about it before so make sure you check those out and the AMX CX in studies has shown increased rigidity in men with a shorter phallus so I think the general take homes about what you should think about with these implants is what do you want to feel comfort in the deflated state do you want to be harder and more rigid in the inflated State and how do you want the pump to feel so generally speaking the AMS implant has a really nice feel when your deflated it feels completely natural it doesn't feel like there's anything in there whereas with the coloplast implant it feels kind of crinkly like there's still some device in the phallus it's usually not a real bother to most people but that is something that people notice as far as inflation and deflation it's really important for you to like the pump you're going to use so when you go to the surgeon ask to see both pumps feel them in your hands see what you like to pump up and to deflate people generally find the coloplast Titan really easy to inflate but a little bit difficult to deflate whereas people who use the Boston Scientific implant find the pump to be a little bit harder to inflate but a lot easier to deflate now the key thing for you to know is that satisfaction is generally above 85 percent for both patients and partners with both implants they are both excellent implants and generally speaking no matter which one you get sometimes it does take some time for patients to get used to it so when you're deciding what is right for you the decision ultimately depends on a few things your individual preferences what you want the implant to feel like inflated or deflated which pump you prefer your Anatomy meaning your Baseline length the girth or any scar tissue or curvature and what your surgeon has the most experience with and prefers regardless of the differences both
