Dialysis or Renal Transplant - What is Right For You? | World Kidney Day Manipal Hospital Dhakuria

hi I'm Dr utan chakrawarti I'm a consultant nephologist and Transplant physician at manipal hospitals dakura today I will speak about the chronic kidney disease the burden in India and what is the way ahead of us so chronic kidney disease is having a prevalence of around 17% in India and the graph doesn't seem to show a dip so while hypertension and diabetes are the key players contributing to the incidence of chronic kidney disease there are certain genetic conditions that also contribute to the same but they are often gone unnoticed so when do you consult a nephologist that's the main question so once you are having diabetes and hypertension you should be doing a routine urine examination and a kidney function test one in every 3 to six months and if that is showing a rise definitely you should be Consulting your nephologist if at all you are having soiling of your feet you're passing reddish coloration of urine or you are having any fro in your urine so that's the point when you should consult your nephologist but if if you're not having any of the comorbidities like diabetes or hypertension but since last two to three months or within a span of six months you are having a decreased appetite you are having this easy fatigability or you are passing more than four times urine at night then also that may Point towards a chronic renal failure and you should be Consulting your nephologist so once you have developed a chronic kidney disease so what is the way forward to us so it is not that even if a every patient developing chronic kidney disease would be needing dialysis or transplantation we look at certain parameters we look at the egfr of the patient to what is maintaining then only we would be counseling the patient for a renal transplantation or to undergo hemodialysis so once you have developed this end stage renal disease so there are two Way Forward of us one is to undergo hemodialysis usually that is two to three times a week or you can undergo renal transplantation so what is more appropriate to us whether we should go for a renal transplantation or we should be opting for a hemodialysis so if a patient is a young patient and you want a healthy lifestyle ahead of you we should always conceal the patient for a renal transplantation you are out of this tight schedule of undergoing two to three times of hemo dialysis per week you are not having all those dietary restrictions ahead of you so obviously you should be opting for a renal transplantation now the next question that comes is who would be donating you the kidneys it can be someone from your family or it can be another one who is not related to you it's not a family member not related to you so with medical advancement uh it has been shown that this blood group barrier is not any contraindication to undergo renal transplantation so people from any blood group can donate you the kidneys uh and if someone from the family is not willing to donate or he's having some of the comorbidities or there are certain limitations to someone who is brain dead he or she can also donate you the kidneys so that is where our state and our country is lacking as of now I would say and there should be a counseling that should be done for cavic transplantation so someone even after death can save multiple lives even after death he can save multiple lives by donating his or her kidneys thank you
