[Music] [Music] Nik hi everyone I'm Dr shumar varali consultant medical gastroenterologist at manipal hospitals Whitefield Bangalore Mr narhari Malik came to us with history of pain abdomen bloating of abdomen and constipation which had progressively worsened in the recent times he also complained of some loss of uptite and loss of weight over a period of time uh he had reduced appetite for his food and reduced taste as well so initially we evaluated him with endoscopy and colonoscopy to hence we had to do a wi endoscopy for him we had taken biopsies from the legion which was suggestive of tubular adenoma with low grade dysplasia after consuling the patient they agreed for endoscopic procedure endoscopic procedure involves endoscopy ampullectomy where when we uh remove the entire illary region with the help of accessories through the endoscope which is usually done under general anesthesia so this procedure comes with certain risks of bleeding whole occuring in the intestine which may require emergency surgery in few cases it also carries the risk of pancreatitis in him we did a endoscopic procedure called as uh endoscopic ampullectomy after doing a endoscopic ultrasound so when we did the endoscopic ultrasound we were able to see that the legion was localized within the ampula and it had not spread beyond the submucosa and because of which we were able to do the procedure endoscopically patient tolerated the procedure well we were able to remove the tumor completely to avoid pancreatis we had put a plastic stent in into the pancreatic duct as well as a CBD stamp post procedure patient had some bleeding which was managed conservatively we did a repeat sheat scan after 5 days to confirm which showed complete removal of the tumor now patient is doing fine both the pancreatic and bent have been removed and he's doing fine and he's on regular followup [Music] [Music]
