hello everybody I am Dr bagat raguraman working as senior consultant cardiologist and program director of heart transplant at Nar Health City bangaluru mital regurgitation essentially means that there is a leakage across the mital valve generally the blood flow is such that the blood from the left atrium flows into the left ventricle and from here it goes into the Iota however if the mital valve is incompetent then as The ventricle is pumping blood into the Iota a significant portion of this blood volume gets back into the left atrium in the reverse direction of what is usually intended there are various ways to tackle mital regurgitation the commonest of course being surgery which has been done from several years so starting from the earlier years where it was complete openart surgery to replace the mital valve then you can now do it through a minimal access approach wherein through a minimal the surgeon is able to replace the mital valve in some centers where the valve is suitable a mital valve repair can also be done however there is a subset of patients wherein a surgical procedure is not feasible either because the disease is too advanced or the patient is very sick and is not a suitable candidate or a high-risk candidate for open art surgery in such unfortunate patients there is now a new Ray of Hope by a procedure called as mitraclip essentially it's an Interventional procedure wherein the axis of approach is through the right groin and then we go across the inal septum and then the device Clips the leaflet so essentially a large and incompetent valve is narrowed down by putting a clip through a Interventional procedure and this Narrows down the mital valve significantly and the leakage is reduced to manageable levels and in a very good procedure it can come down to a trivial level once this is done since the leakage is reduced the patient recovers very well from symptoms of heart failure and becomes ambulatory once again and can lead a reasonably normal [Music] life
